Isfahan Healthy Heart Program (IHHP) was launched in late 1999. After 10 months, its first phases (Situational analysis) was accomplished in 2000. The second (interventional phase) and the third one (Research phase) was finished by2005 and 2007, respectively. The aim of this program was to reduce high-risk cardiovascular behaviors through communicating appropriate health messages to the pubic via means of mass communication and to improve NCDs prevention and promote healthy lifestyle through healthy nutrition, tobacco control, physical activity and coping with stress. It also aimed to assess the feasibility and impact of a comprehensive, community-based approach in a developing country setting.

The communities of intervention were Isfahan and Najafabad counties in Isfahan Province (with a population of 1,900,00) and the Arak county Markazi Province (with a population of 700,000 ) as the reference community.  

IHHP is a comprehensive action-oriented integrated community-based intervention program for non-communicable diseases (NCDs) prevention and control which was implemented in central Iran.

IHHP was conducted by Isfahan Cardiovascular Research Center and Isfahan Province Health Center and was sponsored by the Organization of Planning and Management and the Deputy for Health of Iran Ministry of Health, Treatment, and Medical Education.
